District Nurses

The District Nurse team is attached to the practice. They provide nursing care to patients in their homes.

A large part of the District Nursing role is networking and facilitating care through specialist services, arrange intermediate care provision to prevent hospital admission providing carer support and acting as patient advocate. They will visit you (and your family if appropriate) to discuss and plan the care you need.

The team provides skilled intervention in the areas of palliative care, chronic disease management, care of the elderly and in health promotion.

The Nurse can obtain specialised equipment, including continence products, to help with daily living.

The District Nurses also provide clinics for complex wound assessment/management, Doppler Studies and the assessment/management of urinary/faecal incontinence.
